Quality Improvement Plan

The TAFHT values continuous improvement and strives to deliver quality outcome-based services and programs that meet the needs of our patients. The Quality Improvement Plan is an important part of this process and documents our successes and challenges. Our Quality Management Committee assesses our progress by reviewing our performance statistics to determine if changes are needed. Our goal is to not only ensure that our patients receive the best care possible, at a time when they need it most, but to also ensure that we support and empower our patients to be more engaged in their health and wellness journey.
