Northeastern Ontario Palliative Pain & Symptom Management

As the Palliative Pain and Symptom Management Consultant for the Temiskaming/Cochrane Districts my role is to assist service providers, specifically long term care facilities and community agencies, in the provision of palliative care best practices. This is a free service, funded by the NELHIN and sponsored by VON.

Consultation:

• Help build capacity of service provider by facilitating exchange of information
• Mentor palliative nurses and service providers in all care settings
• Advocate for system development (ie.Use of standardized tools)
• Participate in palliative care and pain & symptom management rounds
• Provide consultation services for service providers caring for clients receiving palliative and end-of-life care

Palliative Pain & Symptom Management

• Assist service providers in the application of the Model to Guide Hospice Palliative Care (HPC) using assessment tools and best practice guidelines
• Consultations available in-person, by telephone, videoconference or e-mail
• Provide case-based education and mentoring for service providers in the delivery of HPC
• Link service providers with specialized hospice palliative care resources across the continuum of care for: LTC homes, Community support agencies, Hospices, Day programs, supportive housing, CCAC and contracted agencies

Education

• Supports standardized palliative care education, including: Fundamentals of Hospice Palliative Care, Advanced Hospice Palliative Care Education, Comprehensive Advanced Palliative Care Education
